I'm feeling ashame to tell that my country's hospital is worst right now... we are forgetting our pride.this is the main problem..even doctors are not having the patriotism of our nation growth..while i'm seeing the peoples in govt hospital... i can find the status of our nation...really all we are working only for task of money not even from the heart...shame on us... Definitely there is no doubt in it infact even some of the private hospitals are in no better shape than the indian govt hospitals. In fact if you get a chance to visit any of these hospitals you will see that the patients lie down on the floor and hold the NS bottles with their one hand

Regarding Govt. Hosp. i say YES as approx 60% Indian population are resides in Rural & 20% are resides in Slums. Where health infrastructure (including Staff Quartersare) so poor. Most of the Doctors are not interested to resides in villages due to lack of basic facilities. Lack of sensitivity regarding Health & Hygiene among the people. Lack of Responsibility among the govt. officials. All these reasons helps Govt. Hosp. to become worst...

As rightly said there is no answer to the debate that the state of Indian hospitals is worst in the country or not worst? As India is huge country with 28 states and the health is state subject; there are stark differences in provision of health care across Indian states. In some states it is worst and in some states it is good (that also in some parts of the states). It is multifaceted problem. In fact I may see it through two angles. One is that if we look at the government hospitals in terms of infrastructure and the 3M (material, manpower money supply) it is worst ever; however there are reasons to make this situation happen worst. India faces a huge need gap in terms of availability of number of hospital beds per 1000 population. With a world average of 3.96 hospital beds per 1000 population India stands just a little over 0.7 hospital beds per 1000 population. Doctors to patient ratio is too scarce as well as medicines are out of stock 24*7? The 1993 World Bank Report,Investing in Health(1) clearly suggestive of denying the comprehensive care to all and only provide the essential care for few medical conditions. This policy was clearly adapted as we can see through successive budgetary allocations to health. There were patients who were forcefully kept far away from availing the basic health care and numbers of “discharge against medical advice” (popularly known among doctors DAMA) cases increased. This word make doctors and health care provider to be not the part of sins they are doing within the system by killing the patients by giving DAMA (in many cases patient did not afford treatment and they ask for the discharge and doctors wrote DAMA). Despite of the worst conditions of the hospital people tend to visit the hospital and avail the services but this is also hampered. Faulty appointment of the doctors on the important positions also has seen the magnificent change on the administration of the hospitals. Before many politicians used to take the treatment from hospitals but now even SHAKHA ADHYASHYA is also not visiting the government hospitals. The political pressure was never built because it was nobody’s issue and nobody’s concern. Politicians and upper class intellectuals’ avail the private up to date services and the opposition party has communal, class and caste issues to fight for. Rest is the ruling government which is a big corporate in itself so it is obvious that why they will look at the loss business (as healthcare is only about spending and there are no returns even though it is built on the public taxes) Quality of care in terms of hospitality (hospital me hospitality?) is the dream which my generation will be hardly able to see in the lifetime. The behaviour of the staff (ward boy to dean sahib) is totally inhumane and giving a care is big favour for them… …To be continued in next post

Hi Bindu, as an italian fellow I'm not in a position to answer to your question. Yet, the problem you're laying down is the problem (to be able to judge the quality of hospital performance) I'm trying to cope with in the last 5 yrs!. As Sanjay says, there are many parameters to be considered. The OMS is trying to define a system of Hospitals assesment (set of parameters), as many other organizations (e.g. Joint Commission international). It would be great to create a website to allow people who experenced hospital intervention to record information about their satisfaction. I'm trying to do that, even if it's not simple. The database is supposed to be very rich of variables and data. Think only about the official list of the names of hospitals (public and private ones) to be loaded in the national databases. This is necessary to have correct statistics afterwards. I'll let you know about the pilot test of Italian prototype and, if you like, we could make some work together for common aspects that could be considerable for an indian version and at international level. I do agree that our Hospitals do not have good infrastructure and service delivery in comparison to western countries. We are still an emerging economy and cannot rely on Government to provide healthcare facilities. There is ample opportunity for Private Hospital chains and Not for profit hospitals to venture into rural sector with low cost biz models.Also Rural road connectivity must be enhanced so that patients can reach to the nearest healthcare centers. If government is inefficient in managing the hospitals it should hive it off to private hospitals on contract basis.

No, it is no so. It is better than several countries of the world. It is also true that Govt.hospitals in India are no better than the private hospitals but you must consider the no. of patients in govt hospitals. One data says that total no. of patients treated in one day in a district hospital is more than what other countries hospitals treat in a month. So, you should look into that facts also.

Look .... this not a 'fair' style to ask. U belong to "Care Biomedicals" and might have some interest/s in hospitals be Govt. or Private. Worst in the world ... ? What promted U to ask like this ... our AIIMs are world famous ... treat patients from SAARC and other countries. One thing is clear .. Govt hospitals may not try to be Private Hospitals. Privates have "profit motives" which is very much absent in case of Govt's. However, who can afford, may easily take services of Private Hospitals.

Folks, here I am extending my thoughts from previous post...... The other angle is just the fine scrutiny of the good things and positive attitude about the future. As I have stated earlier health is state subject, some states are doing well in terms of infrastructure, budgetary spending and the using the NRHM fund in most effective way. NRHM is a flagship program who at least along with its lacunae can provide the hopes and ways to deliverables in various ways. There are ways to strengthen the institutions. As I have seen very objectively at some districts of the Gujarat Government as how they have come up with the NABH accredited hospitals and PHC’s. They have tried to innovate the new ideas to revamp the medical and health services by giving free hands to young, professionally skilled health administers and managers. It is possible and is happening in other states also. The infection control and quality control will be next steps we can see through bits and pieces here and there. We have also witnessing the various groups, civil societies participating in the demanding equitable, affordable and quality health care to the all the population of the country. The Rogi Kalyan Samitis’ in the different hospitals are strengthening and showing their efficacy by doing good administration. Ending on optimistic tone..Folks, if you want a change? Hi Bindu, a statement that all indian govt hospitals are the worst in the world, would be inappropriate. Govt hospitals caters mainly to the lowest rugs of the indian population, and going by the education and awareness status of these population, one can only expect a govt hospital to be shabbily maintained. As Dr. Girish has highlighted there is huge gap between healthcare requirement and the population per se. When you compare with govt hospitals in other countries, please remember there every individual is compulsorily health insured and therefore the govt need not subsidise on everything. therefore they are able to pay more to the work staff which cascades down to a better work respect from every individual (like the one we see in private setups). I have personally seen and have associated myself with a govt hospital being maintained in a manner similar to any private setup that too in B'lore. there the fund allocation is never scarce as it is a Tertiary health care centre and is designated as institute of national importance and shares a world over good repute. To sum it up it has really maintained well both in infrastructure and patient care and management. it would a good point to ponder as to why other hospitals dont do it so? i believe its just need a collective effort both from Staff and patients.

Hi Bindu, Well.... I have not seen any Govt.Hospitals outside Kerala. As far as the hospitals in Kerala are concerned, we cannot say it is worst. Because the foot fall of patients, especially poor patients, is more than that of any private hospital. Though the facilities are not good as private hospitals, given facilities are used to the maximum. Proper treatments are given. Patients are not asked to under go any unwanted tests or scanning unless the Doctor is corrupted and receives any kind of commission from the labs.The Doctors in Govt. Hospitals are well experienced. Thats why they have good demands, even after retirement, at private hospitals. But there is no one take responsibilities for the proper maintenance of Govt. hospitals. Everybody do their duty and go home. People themselves do not obey the rules. People spit and litter on the hospital premises and complains that the hospital is not neat and clean. If these things are taken care, then nobody would say that Govt.hospitals are worst in India.

Hi Bindu, a good debate, but I would like to put my comment against you with due respect for your opinion. I agree that Indian Govt hospitals are lagging behind what we are expecting from them, but how can you say so authoritatively that it is worst among the world with around 192 countries. Any reports or researches on this as a proof. At least some states in India maintaining a descent standard among the hospital. It is better to see some positives rather than descriminating our selves, Sorry if I am wrong.
