According to me if your concrete is already failing, then you have to make a thorough inspection of the situation to find out the nature, extent and cause, of the defect. After you make the inspection, you can drill cores or break or saw off pieces to examine and check in the laboratory and you must decide what to do to the structure. The first question you must ask is: "Can it be repaired or does the structure have to be replaced?" Usually the structure can be repaired. Most problems with concrete can be solved by figuring out a way to keep the water from getting in. Almost every concrete failure can be traced to a water problem.

We can not generliase the situation of falling concrete. Water,dampness,seepage is the main reason leading to corrosion of steel and once steel is corroded concrete leaves the steel i.e Bond between steel and concrete is broken. Concrete can also fall due to cracks due to heavy loading or due earthquake moments. Reapairing is to be undertaken by proper examination of the structure. Rich concrete mix with some bonding agents can be used. As concrete take some time for setting and it is difficult to retain cocnerete specially in the underneath situation like bottom of slabs and beams. In such situations some bonding agents with fast setting properties can be sucessfully used. Once steel get a proper cover and source of seepage and dampnes is checked repaired concrete surface will have good life.
