Most of the Business Process fails due to process complexities, human dependency, improper implementation and consolidation . Process must not rely on human input where majority of people are not intersetd to feed data into process and manipulate only for passing audit. The reality may be too far on which business leaders/Stake holders make decisions and genuinely business fails due to corrupt process. Here comes the need of automation and integrating all organization processes in a single place would only make process smarter and intelligent enough for decision makers.

My stand point on this issue is that most business fails because the organisation fails to buy in the people(staff) into its vision and objectives and so there is always a paradigm shift. People just do things the way they met it without due understanding of why it is done that way.
Autimation will help but the reality is "gabbage in-gabbage out". If the purpose of a thing is not known, abuse is inevitable. The idea has to be driven down into the heart of the people and they should buy-in first.
