The standard practices and checklists are considered theoretical way of working and are avoided in the name of practicality. That is how we work in India. This does not always affect adversly but the chances of unwanted situations increase due to such practices. |

Yes I agree with Mr Vivek Singh, we are not implementing standard practices. Our people always awaits some one will come and resue us. Every body must take the responsibility to implement the standard practice after some time it become normal,
I agree. The standard practices like making of proper Lifting Plans and Rgging Studies for each non standard Lifts and doing proper risk assessments for such lifts could have avoided the Crane's crash tragedy.
I do agree with Vivek this is not a small matter to neglect thinking that it can be rectified later, all the checklist and standard practices designs every thing has to be respected, no deviation all the mix designs should be double checked at every stage. It's not that we don't have the technology we can handle world class projects classic example is Bandra Worli link in Mumbai.

According to me, DMRC is an efficient and an outstanding corporation. But technological failures do happen sometimes, they happen all over the world. It is something to be regretted, it is something from which a lesson can be learnt...In way we should look at it as a setback. Also, there are lot of flaws in our system of Bridge construction. Indian construction is still using old style of constructing bridges and adoption of new technology is minimal. The reason I think may be lack of awareness, fixed mindset and those processes being too much costly.. |

I agree to your views. We Indians are a bit careless and leave the future to good destiny very often. We expect many of our flaws to be taken care of by the prayers we offer every morning ( to Hanuman ji ).
Many of the times, it so happens, but sometimes, when He ( Hanuman ji ) is busy elsewhere, such incidents happen.
It is the call of time, that we ensure NO Deviation from the standard method statements, at whatever ( rework ) cost. This would ensure safety of lives in our country.

General tendancy in our construction industry is to give least importance to the checklist to be follwed and there is lack of constant vigilance in supervision. The instance of the worker pointing out the defect in the pillar and the negligence shown by the engineer in charge as said by pragya all are pointing to this fact. |

Actually what needs to be done is that training programmes should be given to the supervisors/site engineers about the importance of vigilant supervision and the top officials should be ready to recognise and promote people who are dedicated to their work.

Excessive dependence on contractors and sub contractors lead no where.The micro planning now should be order of the day rather than the norm.Each activities should be planned meticulously.The method statement for each activities should be drawn taking care of any uncertainty.The casualties may be prevented with bird's eye on minute details defined like rescue operations and out reach. |

I want to add further that accidents happen as heavy tackles are not properly checked.The health system is not functional at all.Due to flagging health conditions,the reflexes of site workers and supervisors lack intuition.The leadership lack Nelson'eye.The procedures must recognize the role of Top authority down to the individual workers.This is only possible when they are aware of their role and accruing benefits.
As Pragya rightly indicated that fault in the peer was brought to the notice of the site officials.They did not pay heed to the alarm which resulted in the mishap.This very fault line indicated that there is lack of accountability in the system and the innocent workers have to inure the consequences.It is therefore all the more important that rescue operations must be kept uptodate.

We are mostly lacking in using appropriate technology in every feild including construction & Infrastructure building. I conclude the result of every accident is only due to a human error.We as Indians are bit relaxed because we are in best democratic region and era, so face the cosequenes more than others. It is high time to think of appropriate technology of mass production of pre fabricated units in all infrastructure works and transport to site for fixing. It can begin from menufacturing of houses to any other items.It is also necessary to train contineously regarding use of checklist, construction sefty to all concerned irespective of his qualification and post. The detail repot of error at site be diplayed for knowldge of others. |

The main priority to start the projectis.
1. Proper planning and adhere to it.
2. Selction of subcontractor shall be based on the all technical adviser rather than papers/documents.
3 Safety and Qualiy standards must be impliment.
4. Unwanted presure and interferance of politishians leads to failure.
5. 100% transperancy in the all feilds.
6. Necessary checklists prior to start the jobs.

At first, I am not aware of the procedures laid out for each of the major activity of this mega project and the quality control during their implementaion. In my Opinion we are lacking in Stringent Quality Control. If proper procedures and plans would have been done before executing the jobs and thereafter stringent qulaity control would have been there by dedicated people, these lives could have been saved. Definitely India is now reasonably have sound practice of Project/Construction Mnagement in place.But I feel Risk Mnagement which is a very vital component in Project/Construction Management,many of our Practitioners are lacking.Risk assesment at the project initiation stage and review the risk assesment through out the project life cycle is very essential.The risk could be minimised or fully mitigated only by maintaining a risk register which is a dynamic document.The contractors risk register is also a part of the Project Risk register.The regular risk review meeting in particular interval should be conducted by the Risk manager.The owner has to realise this and ensure the risk management team are active in their assigned role.The repeated accidents happened with Delhi Metro as per my view may be a cause of lacking in the above aspect.If risk identified in appropriate time whether it's business risk,contracting risk,execution risk or any associted risk then it can be easily evaluted and minimized or mitigated. |
