As there are various Agile practices which are out there helping out in the development of the software programs, but Agile requires a proper model or structure,  So what are the best practices for Agile modelling??

Agile and entrepreneurship share a pattern to engage closely with the environment (clients) to rapidly

produce most wanted and adjust to suit. Inspect and adapt the process on an ongoing basis. SCRUM is the base model, useless by itself so it has to be populated with useful routines and relationships.

 According to me a Active Stakeholder Participation is required for the success of Agile, where stakeholders should provide timely information, and be as actively involved in the development process through the use of  inclusive tools and techniques.  Operations and support staff also must actively work with the project team towards making the production environment ready to accept the system, other system teams must work with yours to support integration efforts, and maintenance developers must work and adapt to the technologies and techniques used by your system. By this one can have a best model for Agile..

Model Storming

I agree with subhash on Active stakeholders participation, but having model storming is very good, and my experience is that the vast majority of model stroming or sessions involve a few people, usually just two or three, who discuss an issue while sketching on paper or a whiteboard.  These “model storming sessions” are typical events, where one project team member will ask another to model with them, typically lasting for five to ten minutes .  The people get together, gather around a shared modeling tool like whiteboard and explore the issue until their satisfied that they understand it, then they continue on with their work, This model storming is one of the best practices in Agile modelling..
