As we normally do FMEA (DFMEA and PFMEA) while disign and process implementation
but still if we are facing problem of rejection then we have to go slowly towords FMEA
1) Pareto chart
2) After 5 to 8 continuous analysis of Pareto chart then do action on first 3
This is the initial smell of burning
Then u can go for FMEA
