There four types of decision making models. These are:

a) Rational Decision Making Model
b) Creative Decision Making Model
c) Analytical Decision Making Model
d) Admin Decision Making Model

Of the above four, lot of information is available for the first three models but not much about the last model.

I recommend you developing your own questionnaire on Rational Decision making model. This is one if the most suitable decision making model for practising managers. 

I had downloaded questionnaire on Analytical Decision Making Model from the net. But when I used it in my training programme I found that 95% of the participants had "Analytical" decision making style. I did not find participants having other three decision making styles i.e. behavioural, conceptual and directive.

This flaw comes up because of in workplace we are driven by processes. There is measurement of process variation also. Against this backdrop, you won't get many on conceptual or behavioural side. Of course amongst marketing personnel you may get conceptual style.

In my all the training programmes on Decision Making, I told participants to share at least one decision that had gone wrong in the past. I found that effectiveness of all the decisions could be analysed on Rational Decision Making model.

A management leacture heard many years ago. 
Decision making process contains the following three steps. 
First Define the problem. 
Second Detect the alternatives of solving the problem and 
Thirdly choose the right course of action.

Admin Decision Making Model is unique to an organisation.  How can you handle a situation when an employee slaps the administrative head in front of the team!  If this is male hitting male, the decision is different, if a male hits a woman boss, the decision is different and vice versa!  Most of the organisations would like to under play this kind of situation; a publicity of this sort will ruin the future of the organisation!  ICICI can afford to take a call in such a situation, but not the Nationalised Banks, as they have to go through a very rigid procedures!
